About
Our Mission
Strengthening access to affordable community-based health care through advocacy, education and technical assistance.
What We Do
SoonerVerse helps health centers improve quality of care and patient safety by using health information technology to reduce costs and improve care coordination. We provide specialized training and technical assistance to help participating health centers:
- Increase patients’ access to their personal health data
- Improve patients’ engagement with their health care team
- Improve HIT usability to minimize provider burden
- Run security risk assessments and develop risk reduction strategies
- Increase the use of national standards to share patient information securely with key partners
- Use both clinical and non-clinical data to improve and coordinate care
- Improve analytics capacity to support value-based care activities
- Use social determinants of health to improve both population and patient health
Our Funding
SoonerVerse is a Health Center Controlled Network (HCCN) as defined by Section 330(e)(1)(c) of the Public Health Service Act. We are supported by the Health Resources Services Administration (HRSA) of the U.S. Department of Health and Human Services (HHS).
